How to send your X-rays for a free microsurgery second opinion
Why most second opinions are useless
A surgeon asked to comment on "a hand injury" can only reply in generalities. Asked to comment on a specific finger with two X-ray views, a photograph and a date, the same surgeon can tell you whether the bone can be salvaged, whether the soft tissue will cover it and what the operation would involve. The quality of the answer is limited by the file, not by the surgeon.
The checklist
1. Photographs (the part patients skip)
Take them in daylight, against a plain background, with the flash off:
- The whole hand or foot, palm and back
- Close-up of the wound or amputation stump, in focus
- The amputated part, if you still have it, next to a ruler
- Any previous scars, grafts or flaps, from two angles
Do not send photographs taken with a flash at night. Do not crop out the wrist — surgeons use the whole limb to judge tendon and nerve continuity.
2. Imaging
- Plain X-rays, two views minimum (AP and lateral), as original DICOM files or high-resolution JPGs
- CT or MRI if you have had one, including the radiology report
- Vascular studies (CTA, angiography, Doppler report) for limb salvage cases
Screenshots of a computer screen showing an X-ray are close to worthless. Ask the imaging centre for the files on a disc or a download link — you are entitled to them.
3. The history, in five lines
Write it as a timeline, not an essay:
- Date of injury, mechanism (press, saw, crush, road traffic)
- What was done, where, and when
- Current status: is the wound healed, is there infection, is hardware in place
- What you cannot do today (button a shirt, lift a cup, bear weight)
- What you have been told is possible, and by whom
4. Medication and conditions
Diabetes, smoking, anticoagulants, previous radiotherapy to the area — these change what is safe to offer, so list them.
How to send it
Email or a file-transfer link is fine; large DICOM sets are usually 20–200 MB. Do not paste medical records into a public form. Ask before sending who will see the file, and keep your own copy.
What you should get back, within 48 hours: a written opinion naming the likely procedure, whether it is technically feasible, the realistic functional outcome, the length of stay, and a fixed all-inclusive quote. If an answer is vague about feasibility, ask again in one sentence: "Is this operation possible in my case, yes or no, and what would stop it?"
What a second opinion is not
It is not a diagnosis, and it does not replace examination. A surgeon who offers a firm plan without ever seeing you in person is guessing; a surgeon who offers a provisional plan and states what would change it is working properly. Either way, the decision remains yours — and a written opinion costs you nothing to keep on file while you decide.
