Waiting times for replantation surgery: China, the US, Canada and the UK compared
The clock is not the same for every tissue
A finger with no muscle can survive cool ischaemia for many hours; a limb with muscle — forearm, calf, thigh — begins irreversible damage far sooner. That asymmetry is why "wait a few weeks for a specialist slot" is a reasonable sentence for an elective procedure and an unacceptable one for a major limb injury.
So the question is not whether a health system is good. It is: from the moment you are injured, how many days until a microsurgeon opens the vessels?
Typical times from first contact to theatre
- China (Zhengzhou) — 5–10 days for an international patient who has already sent imaging; same-day or next-day for open fractures and acute ischaemia arriving in person
- United States — 3–8 weeks in most markets for non-emergency replantation or flap coverage; immediate only if you are already inside a level-1 trauma centre
- Canada — 3–9 months for scheduled reconstruction through a provincial referral; emergency replantation is faster but concentrated in a few centres
- Western Europe — 4–12 weeks, varying widely by country and by whether your case crosses a regional boundary
The pattern is consistent: emergency care is fast everywhere, scheduled reconstruction is fast almost nowhere.
Where the weeks actually go
- Referral. Your family doctor must be convinced the case is surgical, then finds a hand surgeon who accepts referrals.
- Consult queue. The first appointment is often the long one — 4 to 20 weeks.
- Imaging and clearance. MRI, vascular studies, cardiac clearance, each with its own queue.
- Theatre scheduling. Even after approval, an operating room with microscope time and a free bed has to align.
- Insurance authorisation. In some systems this is the single longest block.
Only step 4 is genuinely about surgical capacity. The rest is administration — and administration is what an international patient pathway is built to bypass.
What you can compress yourself
- Send complete imaging up front. Photographs, X-rays, a timeline. An incomplete file restarts the clock.
- Ask for the plan in writing, with dates. "Reviewed within 48 hours, theatre slot within 5 days of arrival" is a promise you can hold a coordinator to.
- Separate the assessment from the travel decision. A written second opinion costs nothing and is valid wherever you decide to be treated — including at home.
- Do not wait for a scheduled slot if the limb is ischemic. Go to the nearest emergency department first, wherever you are. Replantation after a long delay is possible for digits; it is far less so for muscle.
The uncomfortable part
Waiting lists are not a moral failing of any single system — they are the arithmetic of finite theatre time. But arithmetic has consequences: a finger that could have been saved in week one is sometimes a revision amputation in month four. If you are reading this while deciding whether to wait, the honest advice is to get a written opinion now and keep your options open. It costs you nothing to know.
